At Teesside Hospice, we understand that the journey through illness and care can be filled with uncertainty and difficult decisions. That's why we offer our Outreach Service, bringing the compassionate care you need directly to the comfort of your own home. Our goal is to support you and your loved ones at every stage, providing personalised care and helping you navigate the complexities of your health and treatment options.

Our Outreach Nurse is at the heart of this service, working closely with a team of professionals including our Occupational Therapist, Social Worker, and Physiotherapist. Together, they offer a holistic approach to your care, tailored to meet your specific needs. Whether you need help understanding medical information from your GP, making practical decisions about your care, or just someone to talk to about your concerns, our Outreach Service is here for you.

Through educating and empowering patients just like Lorraine in their care, hear how our outreach service has changed the way she feels about hospice care:

“Hello, my name’s Lorraine Richardson, and I’m just a local girl from Middlesbrough. I’m very very happy to be here, it’s a beautiful place. Under not so much the circumstances, but yes.”

“I don’t want to end my days at home, I want to end my days somewhere here. We had to get Lisa involved, and as soon as she saw me, my leg was really swollen and I was really struggling at home to do just my daily activities really.”

“Lisa came and obviously wanted me to come to the hospice, just for symptom management. When people think of a hospice, they think that’s it, but it couldn’t be further from the truth when you actually get here, it really couldn’t.”

“When I first got here, I must admit it’s the word ‘hospice’ that you hear, but Lisa totally took that fear away. She said don’t think of it as a hospice, think of it as a private, specialist hospital.”

“The nurses could not be more helpful at all. They will do what is needed, at a time when it is needed, but when I need to do it, at my own level and my own pace. If I want to have my breakfast first before I do whatever, yes I can.”

“They are so accommodating with visitors, you can have anybody here, just for comfort for me. They are a massive comfort, but there’s nothing like your friends. If you just need a friendly face, you can pop in at any time, and they will just give you a little chat about everyday life, but they are so cheery and so lift you up. They’re just born to do their job, they really are born to do this job.”

Lorraine's story is a testament to the importance of our Outreach Service. It shows how we can help patients turn anxiety into empowerment and navigate their care journey with confidence and comfort.
