If you or a loved one is living with a life-limiting illness, Teesside Hospice may be able to support you throughout their journey.
A referral can be made by a health professional involved in your or your loved one's care. This may be GP, community nurse, physiotherapist or speech and language therapist.
A health professional is required to complete the referral as we require more medical information. This is helpful in addition to the information patients and families give.

Once you have been referred, your Outreach Nurse will phone you to have a chat about your needs. Sometimes advice over the phone may be all that you require but in most cases your nurse will work with you to arrange a home visit.
At your first home visit, your Outreach Nurse will make a full assessment of your care needs and plan any ongoing support you may need in the future.
For routine referrals, we aim to make contact within 3 days.
Urgent referrals (i.e. requests for admission that day) aim to be addressed the same day.
