Make a Referral for the Inpatient Unit

 

Referrals to Teesside Hospice can be made by submitting the referral form by email to stees.teessidehospice.cas@nhs.net 


 

Referral process

 

Patients may be assessed by Teesside Hospice’s Outreach Nurse Practitioner following a referral that may be in the patients own home, hospital or health care facility. Referrals are discussed by our MDT at hospice daily.

For routine referrals, we aim to make contact within 3 days.                                         
Urgent referrals (i.e. requests for admission that day) aim to be addressed the same day.

Referral Criteria

 

Our criteria for Inpatient admissions include:

Symptom management- The inpatient unit provides treatment for management of pain and other complex symptoms including psychological symptoms. These symptoms cannot be managed in the patient’s current environment, and require specialist support, regular assessment and monitoring. 

End of life care - We will do our best to offer a bed for patients who have expressed that their preferred place of death is at Teesside Hospice. However, due to the demand and bed occupancy this may not always be possible. 

We are a short term hospice and unfortunately do not offer respite services. However, we may be able to support the patient through our other services. If you have any queries about your patient's eligibility, please call 01642 811060 to speak to our team.
