For all referrals please contact the Medical Secretary at Teesside Hospice. You can telephone on: 01642 296927 or you can complete the referral form below and return it to the address stated or via fax to: 01642 823034 (please refer to the Referral Admissions Policy before completing).

Teesside Hospice acknowledges referrals from GP's, Consultants, Hospital Staff, Macmillan Nurses, Community Nurses and other Specialist Nurses.

NEW REFERRAL CRITERIA & REFERRAL FORMS AS OF 1ST AUGUST 2009

Referral Criteria

Referrals can be made for any of the following areas:

Admission
Day Care
Outpatient Appointment
Ward Consultation
Advice
Domiciliary
Bereavement Counselling
Lymphoedema

Patient's referrals can be for any of the following reasons:

Respite*
Social
Spiritual
Symptom Control
End Stage Care
Rehabilitation
Lymphoedema
Bereavement

* Respite is arranged on a non-urgent basis and for a fixed period of one or two weeks
