How to refer

We accept referrals from GPs, Consultants, hospital staff, Macmillan Nurses, Community Nurses and other Specialist Nurses.

If one of your patients needs our expert help please fill in our general referral form below. Here you can fill in any relevant information such as scan reports or blood results. If your referral is urgent please call our Clinical Team on 01642 811060.

Reports

As a healthcare provider we are required by law to publish a Quality Account report and make it available to the public. Quality Accounts are an important way for local services contracted to the NHS to report on quality and show improvements in the services they deliver to local communities and stakeholders.

The quality of the services is measured by looking at patient safety, the effectiveness of treatments that patients receive and patient feedback about the care provided.

Education and training

Teesside Hospice provides an education programme throughout the year on a variety of topics linked to palliative care. This includes full training days for Registered General Nurse’s and also one-hour lunch time education sessions.

The education sessions are presented by experienced healthcare professionals from either Teesside Hospice or other organisations in the locality.

These sessions are available to all Teesside Hospice staff and health care professionals within the South Tees, Hambleton and Richmondshire locality free of charge.
